Fixed-Fee Package · Custom Engagement Proposal

RHC Mock Survey Package

A Rural Health Clinic does not get to choose when the surveyor walks in. Certification and recertification surveys, conducted by your State Agency or a CMS-approved accrediting organization, evaluate your clinic against the Conditions for Certification at 42 C.F.R. Part 491, Subpart A. The clinics that come through cleanly are the ones that found their own gaps first.

This package is a full survey simulation: I evaluate your clinic the way a surveyor will, then hand you the findings, the priorities, and the corrective action plan while there is still time to fix things.

What Is Included

  • Pre-survey document review: policies and procedures, prior survey history, biennial evaluation, and emergency preparedness program documentation
  • Full survey simulation across the Conditions for Certification at 42 C.F.R. Part 491, Subpart A, conducted on site or remotely
  • Structured review of a sample of medical charts for the documentation elements surveyors check
  • Staff interviews mirroring surveyor questioning, so your team has answered the hard questions once before it counts
  • Physical environment and life-safety walkthrough when conducted on site
  • Exit conference with your leadership, same day
  • Written findings mapped to the specific condition and standard each gap falls under
  • Prioritized corrective action plan with owners and timelines
  • One follow-up check-in to review corrective action progress

Regulatory Anchor

Built to the Rural Health Clinic Conditions for Certification at 42 C.F.R. Part 491, Subpart A, verified against the live eCFR text, June 2026. Chart review is conducted under a Business Associate Agreement and the confidentiality protections named in the engagement letter.

Pricing

Fixed fee per clinic, $5,500 to $9,500 depending on clinic size, scope, and delivery model. Fully remote mock surveys sit at the lower end of the range. On-site engagements include one named on-site day and sit at the upper end. Every engagement is custom-quoted: your exact fixed fee, deliverables, and timeline are set in a written engagement proposal before any work begins. Pairs naturally with the Biennial Program Evaluation and Emergency Preparedness packages; combined scopes are quoted together.

What Your Clinic Provides

The Rural Health Clinic agrees to provide all needed documentation and at least one specific point of contact for all coordination. Timelines in the engagement proposal depend on both.

Delivery and Travel

All work is delivered remotely unless otherwise agreed in the engagement letter. When on-site days are agreed, the proposal names the number of included on-site days so travel never sits open-ended inside a fixed fee. Travel is reimbursed at actual cost, with meals at current federal per diem (GSA) rates.
